CUSTOMER / PARTNER IDENTIFICATION:

On my own behalf, as Client / Partner (buyer, lessee, beneficiary, depositor, broker, etc.), in the commercial relationship with ON TARGET PROJECT SRL (INSET SPORTS & EVENTS), as Operator, through this document, being informed by the Operator about my rights regarding data protection (the right to be informed, the right of access, the right of opposition, the right to restrict data processing, the right to be forgotten, the right to data portability),I declare that I agree and freely and expressly agree that ON TARGET PROJECT SRL (INSET SPORTS & EVENTS) may process my personal data under the following conditions:

OPERATOR IDENTIFICATION:

ON TARGET PROJECT SRL (INSET SPORTS & EVENTS) with headquarter in Loc Mamaia, Mun. Constanta, Aleea Siracuza 2, bloc 9, Ap.27, Jud. Constanta, Romania, identified with RO31114714, & with the registration number J13/135/2013.

PERSONAL DATA PROCESSED

– first and last name

– home address

– Personal Identification Number (CNP), identification data and date of issuance of ID, passport, permit, phone number and e-mail address,

– bank account, IBAN code

METHOD OF PROCESSING AND PURPOSE OF PROCESSING PERSONAL DATA

The processing method is storage in paper form and electronically (as appropriate)

The basis and purpose of processing:

– concluding and executing the Contract (business relationship with the Operator),

– fulfillment of a legal obligation of the operator;

– in the legitimate interest of the operator and/or of a third party.

DURATION OF PROCESSING. THE RIGHT TO RESTRICT PROCESSING, THE RIGHT TO BE FORGOTTEN

I agree that the Operator will collect and process my personal data mentioned above for the entire duration of the contract / business relationship or for a period of 10 years (which is longer), unless otherwise provided by law. After this period, I will inform (in writing) the Operator whether it can continue to store and process my personal data or not, asking it to act accordingly in a positive (further processing) / negative (withdrawal of consent) way.

I will also notify the Operator if I decide to withdraw my consent to the processing of personal data before the expiration of the aforementioned period.

I understand that the withdrawal of consent operates for the future, from the date of written notification addressed to the Operator and may be accompanied by the specific notification to be forgotten, if I decide that I will not buy products and services from ON TARGET PROJECT SRL (INSET SPORTS & EVENTS)  or that I will not it enters into other business relationships with it.

If I fail to inform the Operator of the withdrawal of consent, I agree that the Operator will continue to process my personal data until the information / notification is sent.

RECIPIENTS OF PERSONAL DATA. INTENT TO TRANSFER PERSONAL DATA

The recipients of personal data are:

(i) THE OPERATOR

– for concluding and executing the Contract (business relationship with the Operator),

– for fulfilling a legal obligation of the operator;

– in the legitimate interest of the operator and/or of a third party.

– for specific information/offers of new products/promotions, special offers/invitations to events organized by the company/services – by e-mail or by courier or post

I understand that I have the right to give or not my consent to the processing of personal data, including for marketing purposes without negative consequences related to the execution of the business relationship with the Operator. I also understand that in the event that I have expressed my consent, I can change this option at any time thereafter, with the appropriate notification of the Operator.

(ii) OPERATOR PARTNERS

-directly involved in the development of the business relationship

-public institutions and authorities

-partners involved in the observance of the legal obligations and meeting the legitimate interest by the Operator

In this situation, the Operator must notify its partners of my last decision on the processing of personal data.

I understand that the Operator does not intend to transfer my personal data to third parties, other than those mentioned above and with which I have expressly agreed.

OTHER RIGHTS OF THE CUSTOMER/PARTNER WITH REGARD TO PERSONAL DATA

I have been informed by the Operator that I have the right:

1. a) to be informed about and to have access to my personal data processed by the Operator or transferred to the Recipients mentioned above;
2. b) to modify, correct, update my personal data;
3. c) to have my personal data deleted from its records (the right to be forgotten), to withdraw my consent for future processing;
4. d) the right to data portability;

1. e) the right to submit complaints to the Supervisory Authority For Personal Data Processing in Romania regarding the processing of my personal data.
